• Latest General Information Sheet
• Secondary License or Certificate of Authority, If any
• Original and notarized Board or Partner’s Resolution designating CSB as depository bank and the individuals authorized to sign for and on behalf of the entity
• Identification documents for all signatories and the corporate secretary

Sole Proprietorships: 

• Certificate of Registration issued by the DTI
• Original and notarized Special Power of Attorney (for those with authorized representative)

Associations and Unincorporated Accounts:

• Articles of Association
• Constitution and / or By-Laws
• Original and notarized Board Resolution designating CSB as depository bank and the individuals authorized to sign for and on behalf of the entity

FX Dealers, Money Changers, Remittance Agents: 

• Proof of Registration with the BSP and AMLC

Designated Non-Financial Businesses and Professions (DNFBP):

• Certificate of Registration (COR) or Provisional COR (PCOR) with AMLC

• Driver’s License
• Unified Multi Purpose ID (UMID)
• Professional Regulatory Commission (PRC) ID
• Integrated Bar of the Philippines (IBP) ID

Secondary:

• Other Government IDs not mentioned above
• IDs of Government-Owned or Controlled Corporation (GOCC)
• Covered person registered with and supervised or regulated by the BSP, SEC, IC

Foreign nationals:

• Passport or Alien Certificate of Registration (ACR)

Students:

• School ID signed by the school principal or head of educational institution
