Foreign nationals:
- Passport or Alien Certificate of Registration (ACR)
Corporations and Partnerships:
- Certificate of Incorporation / Partnership issued by the SEC
- Articles of Incorporation / Partnership
- By-Laws
- Latest General Information Sheet
- Secondary License or Certificate of Authority, If any
- Original and notarized Board or Partner’s Resolution designating CSB as depository bank and the individuals authorized to sign for and on behalf of the entity
- Identification documents for all signatories and the corporate secretary
Sole Proprietorships:
- Certificate of Registration issued by the DTI
- Original and notarized Special Power of Attorney (for those with authorized representative)
